Located in Jasper, TX our Primary Care Pavilion is led by Dr. Richard Pham. Dr. Pham is a local favorite and he is excited to be back in Southeast Texas serving our local area. Our staff is ready to take care of you when you need it.

No-Show Appointment Policy

We hope this message finds you well. We would like to take this opportunity to remind you of our No-Show Policy.

At Primary Care Pavilion, we value your time and strive to provide the best possible care for all our patients. In order to ensure that we can accommodate everyone, we ask that you notify us at least 24 hours in advance if you are unable to attend your scheduled appointment.

Failure to cancel or reschedule an appointment in a timely manner will result in a No-Show fee of $25 and repeated no shows may lead to cancellation of future appointments.

If you have any questions regarding this policy or would like to reschedule your appointment, please do not hesitate to contact us.

Dr. Richard Pham, MD

Lead Practitioner

Dr. Pham graduated from the University of Texas Medical Branch in Galveston in 1995. He specializes in family medicine. Dr. Pham enjoys his family and going to the gym.

Melissa Woods

Office Manager

Melissa oversees the smooth daily operations of our office. She has 32 years of experience as support staff in the medical field. Melissa enjoys spending time with her family and friends.

Cathy Simmons

Medical Assistant

Cathy has worked as a medical assistant for 35 years. She assists the doctor in all aspects of patient care. Cathy enjoys outdoor activities, family and all her animals.

Haley Miller

Receptionist

Haley is the smiling face you see when you come into the clinic. She assists patients with appointment setting and paperwork. Haley has 3 children, and enjoys being involved in their activities.

Jasper Hospital District Administrator

Paula has worked in the healthcare industry for more than 30 years. She has extensive experience in accounting, management, information technology and payroll. In her downtime she enjoys spending time with her family and friends.
